    Valid for one month
    • One Time Home Visit (S. Tampa and Whesley Chapel only)
    • Applicable to Any Family Member Aged 18 or Younger
    • Sports Physicals Can be Included as a Visit
    • Procedures and Point-of-Care Testing (Coming Soon)

    650$
    Every month
    +$50 One Time Registration Fee
    Limited Spots
     
    • Monthly Allowance of 3 Credits for Telemedicine Visits
    • Monthly Allowance of 2 Credits for Site Visits
    • Credit Applicable to Any Listed Member Aged 18 or Younger
    • Sports Physicals Can be Included as a Visit
    • Direct Text Messaging with a Physician 24/7
    • Procedures, and Point-of-Care Testing (Coming Soon)
    • Monthly Allowance Credits Can Be Used for Parenting Advice
    • Services exceeding the monthly allowance will be discounted
